Purdue Performance Personalised PREMIUM Agreement
Welcome to PURDUE PERFORMANCE and congratulations on beginning your bespoke Fitness and Running program with the Team Purdue Performance Personalised PREMIUM Package. We are very happy you have chosen PURDUE PERFORMANCE as part of your path to improved running and performances. With the help of our team at PURDUE PERFORMANCE you will improve your knowledge and ability in reaching goals in and around fitness and running.
This Agreement is made and entered into on the by and between PURDUE PERFORMANCE LIMITED and:
The guidelines outlined below are to ensure that the relationship between PURDUE PERFORMANCE and Client and the responsibilities of both parties are clearly appreciated and understood.
PURDUE PERFORMANCE and their assigned coaches will provide a Questionnaire / Fact Find to each new athlete on the Base Package. This will act as an introduction between client and PURDUE PERFORMANCE. Designed to act as a fact find on each client so all training programs can be individually tailored.
PURDUE PERFORMANCE and their assigned coaches will use their skills and knowledge to design a safe and tailored programme of Fitness/Running, they will take into account each clients lifestyle, personal goals, fitness and health status and medical history.
PURDUE PERFORMANCE and their assigned coaches require minimum 7 working days from initial client questionnaire answer session and receiving payment before the clients first training schedule will be set. However, PURDUE PERFORMANCE aim to have all training plans ready at least 1 day before the agreed start date.
PURDUE PERFORMANCE advise you seek medical advice from a doctor before starting any personal fitness/running programs.
Each program is not intended to diagnose, treat, cure or prevent any disease.
PURDUE PERFORMANCE or any of their assigned coaches is not liable for any illness, death or medical reaction caused (before, after and during) by participation in their set training programs.
PURDUE PERFORMANCE and their assigned coaches care personally about every client they have and will work individually with each person to ensure their personal training and running programs are fitting and satisfying.
Payment and personal questionnaire must be received from the client before PURDUE PERFORMANCE will proceed with the personal training programs.
You understand that the results of any fitness/running programme set by PURDUE PERFORMANCE cannot be guaranteed. Your progress depends on your effort and co-operation in and outside of the sessions and your adherence to, and feedback about, the online plans. In particular you acknowledge that individual results may vary, and no particular result is guaranteed.
It is understood that you have to be 100% committed to PURDUE PERFORMANCE and your tailored running program in order to optimise your results and fitness.
You are required to give personal and honest feedback to PURDUE PERFORMANCE on your monthly training program. This will allow the assigned coach to fully review the month and then amend the training to suit for the following month.
Clients must communicate any discomforts, pain or concerns experienced during, pre, post or arising from any sessions or training set through the feedback on the training app or if slightly more important please email on firstname.lastname@example.org.
The client acknowledges that he/she is in good health and physically able to participate in a personalised running program. By signing below, client acknowledges and agrees that he/she has no limiting health conditions that would preclude participation in an exercise program and will immediately inform the trainer if such health condition changes or arises during the client’s participation.
The client understands that there are inherent risks in participating in a program of strenuous exercise. If you sustain or claim to sustain any injury while participating in training, you acknowledge that PURDUE PERFORMANCE is not responsible or liable for any injuries sustained under their program guidance or any medical bills needed caused by injury. The client understands they must state in the initial question and answer session any specific requirements they have with fitness / health or running training example: they have a sore knee so doing certain gym exercises would not be prescribed.
PURDUE PERFORMANCE requires a standing order or direct debit to be set by each client. This payment will be set up via their online payment system on sign up through www.purdueperformance.com.
Payment will start monthly from the selected start date 1st or 15th of the month. Unless selected payment will not be taken on the date on sign up and questionnaire completion.
Client information and records are confidential unless I receive your advance permission to disclose or except as required by law. All of our conversations and information exchanged is further confidential.
The client understands all programs set and conversations has with PURDUE PERFORMANCE are confidential.
All information provided by the client will be treated securely in accordance with the data protection act 1998.
With the Purdue Performance Personalised PREMIUM Package there is no minimum contract length. All Purdue Performance ask is that they are given a 1 months’ notice period. Please email Purdue Performance on email@example.com or let them know directly on your personal message group your request of cancelation.
Each cancelled client will be taken off all systems on the last day of their handed notice month.
There is no automatic right to refund when you wish to cancel any program.
PURDUE PERFORMANCE shall not be liable for the following:
Death / personal injury / illness sustained by the client while participating pre exercise, during exercise or post exercise whilst following their PURDUE PERFORMANCE online training program.
Any marketing, educational or other materials, including PURDUE PERFORMANCE programs, concepts, business model and/or any variations.
The client agrees to use materials solely for their own personal use and development and not to copy, share, publish, sell or reproduce any such materials supplied from PURDUE PERFORMANCE.
If the client wishes to withdraw from their PURDUE PERFORMANCE program they further agree to not passing on any materials supplied after the program termination.
The client understands that although PURDUE PERFORMANCE takes all due care and consideration when designing, setting the online programs and giving running/fitness/injury or nutrition advise, they will not hold them responsible for any injury, death or illness sustained while under their guidance and training plans. The Client declares that they consider themselves suitably healthy to undertake physical exercise and specifically running training. They understand the trainers have a personal back ground in international running and will prescribe advise and tailored programs from their knowledge and experience as top-level athletes and not from specific qualifications linked to fitness and training.
I understand that I undertake any advice or training given at my own risk. By signing this declaration, I accept all the above terms and conditions.

If there are any changes to the medical condition of the client, or to prescribed medication, this should be brought to the immediate attention of Purdue Performance. Failure to do so could affect the suitability of the training set. If any unusual symptoms occur in the client’s physical condition while undertaking a training session, the client must stop exercising immediately and inform Purdue Performance.
